If you are on regular medication, these details are stored on our computer system and you'll be provided with a printed re-order slip listing your medication. You must indicate what you require by ‘ticking’ items you wish to order. Hand in this slip to us 48 hours before your prescription is due. Alternatively, send your request to the surgery with a stamped self-addressed envelope enclosed for us to return the prescription to you. Ensure you post your request with plenty of time for us to return it. You can arrange for your local Pharmacy to order your prescription for you. Emergency treatment can be requested over the telephone, or at reception, from 9.00 am to 12.00 noon or 2.00 pm to 5.15 pm for collection the same day. |
Practice Policies Confidentiality We ask for information about you so that we can ensure you receive the best possible care and treatment. These details are kept to ensure your records are accurate and up-to-date. Records are held both manually and on our computer database, which may be accessed by authorised personnel only. Our staff provide a confidential service to all our patients, and your personal health information will not be disclosed to a third party without your prior consent in writing. All staff are bound by the Data Protection Act 1998 and the Access to Information Act 2000. Complaints Procedure We always try to provide the best service possible, but there may be times when you feel this has not happened. We do have a Practice Complaints Procedure but we are not able to deal with any legal liability or compensation questions. Should you need to make a complaint, please telephone or write to our Practice Manager. Please note that we have to respect patient confidentiality and a patient’s consent will be necessary if a complaint is not made by the patient in person. |
