Your Responsibilities

Please be courteous to all our staff. We are responsible for the care of our employees and as such, violent or abusive behaviour will not be tolerated. Any patients who behave in this way towards the doctors, staff or other patients whilst on the premises will be immediately removed from our Practice list.

It is your responsibility to keep your appointments and, if you cannot, then please inform the surgery immediately so that your appointment may be given to another patient.


All new patients are required to have a full check up and fill in a questionnaire before registration with the Practice. You can register by completing a registration form (GMS1) available at reception, or with your medical card. Once you are registered, in order for us to maintain up-to-date and accurate records, please notify the Practice of any changes of name and/or address as soon as possible.
